Faculty Request Makeup Exam Form
The following form needs to be downloaded to you computer, filled out, and either e-mailed along with the make-up exam to Karen Royster-James, kroysterjames@colum.edu, or delivered in person to the Learning Center if you want to give a proctored make-up exam at least two days prior to the exam date. The document is listed below as a Word document and as an RTF document.
Proctored exams may only be arranged through Karen at the Learning Studio - the Science and Math main office will not proctor exams. Requests for this service must go through the Learning Studio.
