Application

* The New 2009-2010 SiLA Application is now available as an Interactive PDF.

You may fill it out and sign electronically, or print and return the form to the addresses provided on the left.

You should attend an Information Session.

The dates and times are listed here. If you have missed the dates available, please contact your department's representative (listed on the left) for more information.

Please, carefully review the requirements and prerequisites for acceptance.

  • Failure to submit a complete application that contains all the required documents may result in your NOT being accepted to the program.
  • Fill out the form completely. If printing, print neatly, please.
  • You should complete an academic audit on OASIS before applying for SiLA.
  • If you plan on graduating from the program, you must apply for graduation at the time of application.
  • Attach a brief (1-2 pages) essay telling us why the SiLA program is important for your education and career. Describe what you hope to get out of the program and what your career goals are. You should not describe how wonderful the program is. We already know this. We are trying to find out about you as a person, your maturity and your goals. This material should exhibit your ability to present material professionally and should also give some indication of your creativity.
  • Attach a brief resume.
  • Do not use family members as references.
  • Make sure you state which section you wish to attend.
  • You must submit separate applications if you are applying to more than one session.
  • Applications should be returned to the person associated with the program to which you are applying (see list at left).
  • Copies of applications should also be submitted to Don Smith.
  • Students applying for producing, screenwriting or adaptation sessions may submit applications via e-mail as described on the website. Application forms suitable for printing may also be found at the website. In addition, applications are also available at the Reception Desk in the Film & Video Department, Suite 301, 1104 S. Wabash Ave.
  • After reviewing the application, we will schedule an interview for those candidates who qualify.
  • If you are NOT accepted you will be notified and given the reasons and you may reapply for a later date.
  • If you are accepted you will need to register during early registration. It is your responsibility to clear any restrictions prior to registration.
