To initiate a new job with Creative Services, you must first complete one of the following request forms. These are available for download by clicking on the links below. Request forms can be filled out and saved on your computer using Adobe Acrobat Pro and e-mailed to Edward Thomas at ethomas@colum.edu, or printed out and turned in to the Creative Services office as hard copies.
