2008-2009 Verification

  • Verification of your FAFSA (Free Application for Federal Student Aid) is the process of confirming that the information reported by the student/parent (Dependent) or student/and or spouse (Independent) on the FAFSA is accurate.  Approximately 30% of student applicants nationwide are randomly selected by the government for this process.
  • All Columbia College Chicago financial aid applicants selected by the federal government for verification and are registered for courses are required to provide the following documentation prior to the disbursement of any federal or state aid:

  • Dependent Students
  • Independent Students
  • Student/Parents signed 2007 Federal Tax Returns
  • Student and/or Spouse signed 2007 Federal Tax Returns
  • Illinois Residents Only: Parents signed 2007 State Tax Return Or copy of parents IL Drivers License/State ID
  • Illinois Residents Only: Student signed 2007 State Tax Return Or copy of student's IL Drivers License/State ID issued prior to Sept. 1, 2007

 

Verification Worksheet

  • Dependent and Independent students must complete the Verification worksheet in its entirety and it must be signed and dated by both student/parent and or spouse.  Students who have submitted incomplete worksheets will be notified by letter at the address provided on the worksheet.

Copies of Federal Tax Returns

  • Student/Parent(s) and/or Spouse must provide a signed copy of their 2007 Federal Income Tax Returns.  Federal Tax Returns includes the following:
  • Federal 1040, 1040A, or 1040EZ; all returns must be signed by the taxpayer.
  • Proof of Illinois Residency (Illinois grant recipients)

Parent(s) of Dependent Student(s) 

  • Photocopy of Parent(s) signed 2007 IL Tax Return (Or) Valid IL Driver License/ID

    Independent Students

    • Photocopy of signed 2007 IL Tax Return (Or) IL Driver License/ID (dated prior to September 1, 2007)
    • Individual students, depending on their situation, may be required to provide additional documentation to complete the verification process.
    • All students selected for the verification process will be sent a separate communication outlining the process and the specific documentation needed to process their financial. Students who do not submit necessary verification documentation before the required deadlines will have their financial aid award nullified and will be responsible for any charges accrued during the enrollment period.

    • To view what documents are needed to complete your financial aid file please log in to your OASIS account and look under "Documents Needed" under the Financial Aid option.
