How To Get Your 1098-T Form

Attention Students!

• 1098T forms for the 2007 tax year will be ready to view and print January 25.  Hard copies will be mailed the end of the first week of February 2008. If you would like to print your form or if you misplace the hard copy that will be mailed to you, please follow these steps to retrieve your form online:
• 1. Go to the Website
• 2. Click on "Access My Records"
• 3. Enter your social security number or OASIS student ID number and last name. If you have a PIN from last year enter your PIN. If you do not have a PIN skip it and press " Login"
• 4. Now enter your first name, mother's maiden name, and your date of birth. Click " Next"
• 5. If you did not have a PIN. You will need to create a PIN for yourself. It is four numbers of your choice. Click "Finish"
• 6. Now you are in your record and will be able to view your specific information.
