The Hillary R. Kalish Scholarship application is available online through your OASIS portal. Only complete scholarship applications submitted by the deadline will be reviewed by the scholarship committee.
Please follow all directions carefully. Students must submit the following materials to make a complete Hillary R. Kalish Scholarship application - there are five parts to the application:
PART 1: ESSAY
Provide a brief description of your educational and vocational goals. Describe how your medical condition has affected your pursuit of these goals.
You may either write your statement directly into the text box provided on the application form, or cut and paste your statement into the text box from a word processing program such as, Microsoft Word. TIP: When cutting and pasting text from a Word document to the text box, please review for correct formatting and punctuation.
PART 2: ACHIEVEMENT LIST
Submit a list that describes any additional materials or information that supports your application, such as awards you have won; involvement in extracurricular activities, groups or clubs; service to your community; etc.
You may either write your statement directly into the text box provided on the application form, or cut and paste your statement into the text box from a word processing program such as, Microsoft Word. PART 3: LETTER OF RECOMMENDATION
One letter of recommendation from a faculty member at Columbia is required to complete your application.
For your convenience, the online application provides the option of sending an e-mail notification directly to the person you've chosen to complete your recommendation. This person will be invited to complete the recommendation online with ease and will not need to mail, fax, or e-mail their recommendation. Please be prepared with the e-mail address of the person making the recommendation.
Note: Remember to contact your recommender directly before providing his/her information on the application. Be sure to give faculty members as much time as possible to write the letters. Requests made at the last minute are often difficult for faculty to fulfill by the deadline.
PART 4: PHYSICIAN'S STATEMENT
The application requires a signed statement from your physician describing your medical challenge and how it will affect your capacity to attend Columbia. Please download and print the Physician's Statement form here, fill out the top section on page 1, and then send the form to your physician. Once your physician has completed and signed the form, please return it to:

NOTE: The Physician's Statement must be returned to the Office of
Enrollment Management by February 20, 2012 in order for your application to be considered complete.
PART 5: STUDENT AID REPORT
A current Student Aid Report (SAR) must be on file. To receive an SAR, students must complete a financial aid application (FAFSA), available after January 1st of each year.

There are two ways to locate the Hillary R. Kalish Scholarship application:
- Applicant Portal: You may scroll through the list of scholarship programs until you locate the Hillary R. Kalish Scholarship, and then select the "Start Application" link.
- Scholarship Search: Select the "Search Scholarships" link in the navigation on the left-hand side of the page. You may search for the Hillary R. Kalish Scholarship by using category, keyword, or description/name search. Once you locate the program, select the "More Info" link to be taken to the application form.
