Sample Student Organization Constitution

Proposal- Forms and Guide Sheets.  These forms are used to:request Funds, Meeting and Event Space, Catering, and Reimbursement Pre-Approval. Proposals should be submitted at least 3 weeks prior to event, or date that payment is needed.

TRAVEL FORMS

Travel Request - This is the additional paperwork that needs to be completed for any organizations requesting out-of-state travel, vehicle transportation/rental, and plane or hotel reservations.

Travel Waiver - This is the additional paperwork that each individual that is traveling must complete and submit with the Travel Request.

Travel Policy - This is an informational document that all student organizations must be familiar with and use as a resource when planning travel.

OTHER IMPORTANT FORMS

Entertainment Contract - This contract should be completed by any artist, performer, or individual providing services to an organization and is seeking payment. This contract should be signed and submitted with the Proposal prior to the date of service.

Venue Request - This is the additional paperwork needed to request any [C]Space location (Hokin Annex, Hokin Gallery, C33 Gallery, Conaway Center).

Release form for online images - This is the form that students must sign agreeing to release their images for online use.
