Missing Documents 2013-14

This informational page should be used as a directory of missing items students may be required to complete to secure their funding. The directory is organized in alphabetical order and will include a document description as well as instructions on how to fulfill the requirement. Some missing documents may also include a hyperlink to additional information. Please reference the outstanding documents in your OASIS Portal Missing Documents for a list of items required to secure your funding.

Bankruptcy Documents

The National Student Loan Data System (NLSDS) indicates that you have filed bankruptcy and possibly included one or more federal student loans in the bankruptcy claim. A review of your aid history will be conducted to determine your aid eligibility. You will be notified if you are found ineligible for federal student aid.

Dependency Documentation

Your Student Aid Report (SAR) indicates that you answered yes to one or more of the dependency questions 52 - 57 in section three of your 2013-14 Free Application for Federal Student Aid (FAFSA).  In order to continue the processing of your financial aid file, you must  submit the documentation that corresponds to the question(s) to which you answered ‘Yes’.  Click here for a full list of acceptable documentation.

Dependency Status Update

Your Student Aid Report (SAR) indicates that you have made a change to your marital status and marital date on your Free Application for Federal Student Aid (FAFSA).  In order to continue the processing of your financial aid file, you must do the following:  1) Make an appointment to meet with a Student Financial Services Counselor, 2) discuss you aid eligibility as a married student.

Discharge-Loans

Your Student Aid Report (SAR) indicates that your previous federal student loan was discharged due to disability. To receive further federal loan funds you must submit:
 
  1. Certification from a legally licensed physician stating that your condition has improved and that: a) You have the ability to engage in substantial gainful activity; b) You can attend school.
  2. A signed statement that you are aware that a new student loan cannot later be canceled on the basis of any present impairment unless that condition substantially deteriorates and the definition of total and permanent disability is again met.
  3. A letter from the US Department of Education indicating your loan(s) has/have been discharges.

Note: a prior loan discharge does not affect your eligibility for federal grant funds.

Drug Question

Your Student Aid Report (SAR) indicates you reported that you have been convicted of possessing or selling illegal drugs while receiving federal student aid.  If this was an error, please make the necessary correction to your Student Aid Report (SAR).  If you have been convicted of possessing or selling illegal drugs while you were receiving federal student aid, you are NO longer eligible for federal financial aid for the 2013-14 academic year.

High School Completion Status

To complete your verification requirement, please submit a copy of a completed Verification of High School Completion Status form along with the required documentation. The information being requested could not be verified using the documentation submitted as part of your admissions process. Please contact our office to receive clarification regarding the required steps needed to continue the processing of your federal financial aid.

High School Transcript

You have not yet submitted your final high school transcript.  In order for Student Financial Services to be able to disburse your financial aid and/or to receive clearance to register for your second semester of study at Columbia, you must submit your official final high school transcripts that shows your graduation date.  Please submit your transcript to Undergraduate Admissions, Columbia College Chicago, 600 S. Identity/Statement of Educational Purpose

To complete your verification requirement, you must complete an Identity and Statement of Educational Purpose form. This form is only accepted in person or by mail. To submit this form in person, you must contact our office to make an appointment with the Special Counseling Coordinator. Or to submit this form by mail, it must first be completed and notarized by a Notary Public. This form will not be accepted via fax, Electronic Document Submission, or in office drop box.

IRS Transcript Required

It has been determined the 2012 tax information on file is incorrect/invalid or unacceptable for verification purposes.  Please check your OASIS student portal to receive clarification regarding the required documentation needed to continue the processing of your federal financial aid. Loan Default Clearance

Your Student Aid Report (SAR) indicates that you are in default on a federal student loan.  In order to regain federal student aid eligibility you must submit documentation from the Department of Education or a letter on institutional letterhead that your loan is either paid in full, consolidated or is meeting satisfactory repayment arrangements. For more detailed information regarding your loan default, please visit the National Student Loan Data System (NSLDS) for Students website or call 1-800-433-3243.

Overpayments

Your Student Aid Report (SAR) indicates that you owe a federal overpayment. Based on federal regulations, you are not eligible for federal student aid until the overpayment is repaid in full. In order to regain federal student aid eligibility you must provide documentation from the Department of Education or a letter on institutional letterhead indicating your overpayment is either paid in full or that you are making satisfactory repayment arrangements. To complete your verification requirement, please have your parent complete the IRS Data Retrieval process at fafsa.ed.gov or submit a complete copy of your parent(s) 2011 1040, 1040A or 1040EZ Federal Income Tax Return Transcript. Your parent(s) can request a 2011 Federal Income Tax Return Transcript at irs.gov

Proof of Citizenship

The Social Security Administration (SSA) or the Department of Homeland Security (DHS) was unable to confirm your citizenship status. Before you can be considered for federal student aid, you must submit one of the following documents: Birth Certificate, U.S. Birth Abroad Certificate, U.S. Passport, Certificate of Naturalization or Certificate of Citizenship, Alien Registration Card, Permanent Resident Card or a Machine Readable Immigrant Visa (MRIV).

Columbia College Chicago has received your 2013-14 Student Aid Report (SAR) but cannot award you financial aid because your application was rejected.  Please review the comment(s) section of your Student Aid Report (SAR) by logging into fafsa.ed.gov and selecting ‘View Processed Information’.  Please contact our office to receive clarification regarding the required steps needed to continue the processing of your federal financial aid.

Renewal Dependency Override

You were granted a dependency override for the prior financial aid award year.  In order to continue to process your financial aid application with the same status as the prior year, you must submit a Renewal Dependency Override form.  If your situation has changed, you must contact Student Financial Services to schedule an appointment.

Selective Service Registration

As a male between the ages of 18 and 25 you are required to register with the Selective Service in order to receive federal financial aid. Your Student Aid Report (SAR) indicates that you are not registered with the Selective Service. If you believe you have already registered or are exempt, please contact the Selective Service at 847-688-6888 to resolve any registration issues. You may also visit them online at sss.gov to register. Once you have confirmed your status you must submit official Selective Service Documentation of your registration status (i.e. Selective Service Registration Acknowledgement Letter).
 
Note: females and males born before 1960 are not required to register with the Selective Service

SNAP Benefits

Please submit official agency documentation attesting that SNAP benefits were received during 2011 or 2012. Please refer to your student LOOP email to receive specific details regarding the required documentation.

Social Security Number, Name, or Date of Birth Mismatch

The Social Security Administration (SSA) was unable to verify the Social Security Number used to complete your 2013-14 Free Application for Federal Student Aid (FAFSA).  Please submit a copy of your social security card to SFS.
Your Student Aid Report (SAR) indicates that the name and/or date of birth you reported does not correspond with the social security number you reported.
  1. If the name and social security number shown on your SAR are correct, you must contact the Social Security Administration Office to resolve this problem. In the interim, you may provide a copy of your signed social security card as proof your SAR is correct.
  2. If the date of birth is incorrect on your OASIS account, please to go the Records Office to correct the date of birth and bring a copy of your license/ state ID.
  3. If the date of birth is incorrect on your SAR, log onto fafsa.ed.gov and make the necessary corrections to your SAR.

Your Student Aid Report (SAR) indicates that you did not provide parental data on your 2013-14 Free Application for Federal Student Aid (FAFSA).
