If your Campus Card is lost or stolen, contact the Campus Card Office immediately or visit the Online Card Office to suspend your card.
Please note that the replacement fee for lost, damaged, and stolen cards is $25. You may pay your replacement fee online with a credit card or debit card using this secure form. Please print the receipt before you come in as proof of payment. There is no service fee to use this service. You may also pay cash at the Cashier Office before receiving your new ID. The Cashier is located next door to the Campus Card Office at 600 S. Michigan Ave. on the 3rd floor.
When you come in for a replacement, your Columbia Cash balance from your old card will be applied to your new one. To pick up your replacement, you must bring your OASIS ID number, a government-issued ID (driver's license, state ID, passport, or military ID), and a cashier receipt.
A new card will be issued at no charge for students who show proof of a legal name change or a new preferred name, or who have changed enrollment status (i.e., undergraduate to graduate).
