Immunization

Illinois Public Act 85-1315 requires that all students born on or after January 1, 1957 who enroll at least half-time (6 credit hours or more) in an Illinois post-secondary institution (college or university) must prove that they have immunity from certain communicable diseases: diphtheria, tetanus, measles, rubella, and mumps. Proof of immunity must be on file at the institution the student is currently attending.

Please Note: If you do not have a complete immunization record on file by the end of the eighth week of each semester (not including the Summer semester) a registration hold will be placed on your academic record, and you will not be eligible to register for the following semester.

If you have not yet submitted your required immunization records, you may do so by mailing or faxing them to:

Records Office
Columbia College Chicago
600 S. Michigan Avenue
Chicago, IL 60605
Phone: (312) 369-7769
Fax: (312) 369-8091 or (312) 369-8073

In many cases your high school or physician will have your immunization history on file. You may request to have them sent directly to the address above. If you are unable to obtain your records, you must be re-immunized. The Chicago Department of Public Health will provide vaccinations at its immunization clinics. A current list of clinics can be found at the Chicago Department of Public Health website or call the Department of Public Health at (312) 746-5380.

According to the Centers for Disease Control and Prevention (CDC), college students living in close quarters, especially freshmen, may benefit from receiving the new meningitis vaccine, Menactra. It is advisable that you receive this vaccination from your physician at home, prior to coming to Columbia. If you are in the Chicago area and need a referral for the meningitis vaccine, you may contact the Student Health Center at (312) 369-6830 for a list of local health care providers.
