Dependency Question 2012-2013

If you answered YES to certain dependency questions on the 2012-13 Free Application for Federal Student Aid (FAFSA), you will also need to provide documents to verify these data elements entered on your FAFSA even if you were not selected by CPS or Student Financial Services (SFS) for the verification process. 

What FAFSA Questions Require Additional Documentation?

If you answered YES to questions 52-57 on your 2012-13 FAFSA you will be required to submit documentation to Columbia College Chicago to complete your financial aid file. You can view your Student Aid Report (SAR) online at www.fafsa.gov by clicking on ‘View Processed Information’.

Note: you must disable any and all pop-up blockers in order to view your SAR. Once you have identified the question to which you answered YES, locate this question below in order to determine what documentation you are required to submit to SFS.

How Will I Be Notified to Submit Documentation?

All students required to submit dependency question documentation will be notified via Missing Document email to their LOOP account.

What Do I Need to Submit?

The 2012-13 FAFSA Questions are listed below with the corresponding documentation you must submit. If you answered YES to any of these questions in error, you should make a correction to your SAR.

Question 52:

At any time since you turned age 13, were both your parents deceased, were you in foster care or were you a dependent or ward of the court?

Question 53:

Are you or were you an emancipated minor as determined by a court in your state of legal residence?

Question 54:

Are you or were you in legal guardianship as determined by a court in your state of legal residence? For this question, the definition of legal guardianship does not include your parents, even if they were appointed by a court to be your guardian. You are also not considered a legal guardian of yourself.

Question 55:

At any time on or after July 1, 2011 did your high school or school district homeless liaison determine that you were an unaccompanied youth who was homeless?

Question 56:

At any time on or after July 1, 2011 did the director of an emergency shelter or transitional housing program funding by the U.S. Department of Housing and Urban Development determine that you were an unaccompanied youth who was homeless?

Question 57:

At any time on or after July 1, 2011 did the director of a runaway or homeless youth basic center or transitional living program determine that you were unaccompanied youth who was homeless or were self-supporting and at risk of being homeless?
