Immunization

Columbia College Chicago is required by law to comply with the guidelines set forth by the College Immunization Code (77 Ill. Admin. Code 694,) which requires all students born on or after January 1, 1957, who enroll for six credit hours or more to provide the college with proof of immunity against certain communicable diseases.

If you do not have a complete immunization record on file, a registration hold will be placed on your account and you will not be eligible to register for the following semester.

Beginning May 12, 2023, there will be no formal COVID-related requirements on campus or at Columbia-sponsored activities off campus.
  • Immunization Requirements
    • Three (3) tetanus/diphtheria/pertusiss (DPT, DTP, Td, DTaP, or Tdap) boosters. One must be Tdap. One must be within the last 10 years.
      • Note: Tetanus Toxoid (TT) does not meet this requirement.
    • Two (2) measles, mumps, rubella (MMR) vaccines given after one year of age and after December 31, 1967.
      • OR if measles, mumps, or rubella vaccines were given separately:
        • 2 measles immunizations given after one year of age and after December 31, 1967
        • 2 mumps immunizations after one year of age.
        • 2 rubella immunizations after one year of age.
      • OR a positive blood titer test result for measles, mumps, and rubella
        • If you were previously vaccinated, or were exposed to the disease and believe that you now have immunity, and are unable to provide immunization records, a positive titer result is acceptable as proof of immunization. A copy of the lab report must be submitted.
    • One (1) meningococcal conjugate vaccine for students who enter Columbia in Fall 2016 or later, and are age 21 or younger. Must be taken on or after 16th birthday.
      • Note: Menomune and Meningitis B do not meet this requirement.

  • Submit Your Immunization Records

    Please note: If you were vaccinated on Columbia’s campus for COVID-19, the college already has these records and you do not need to submit them again. Please retain your vaccination record card for your records. You are welcome to upload a copy of your vaccination card to your Med Proctor account if you wish.  

    Columbia College Chicago is partnering with MedProctor to submit your immunization records. 

    • Once you’ve confirmed your enrollment to Columbia you will have access to MedProctor. 
    • When you click on the MedProctor link, you’ll be taken to your single sign-on page to log into your Office 365 account if you aren’t already logged in. 
    • Complete your MedProctor profile by entering your personal information.  
    • To submit your COVID-19 vaccination records, simply upload an image of your vaccination card. 
    • For all other immunization requirements, download and print the Immunization Certificate form 
    • Take the Immunization Certificate form to your medical provider for completion 
    • Upload an image of the completed Immunization Certificate form to MedProctor

    Columbia College Chicago does not release immunization records. We recommend that you keep a copy of any documents you submit.

    Students starting at Columbia in Spring 2017 and earlier 

    Your records are not viewable online. If any of your vaccinations expire and require additional vaccinations, you will be emailed before a hold is placed on your account.

    Columbia College Chicago does not release copies of your medical records to students or any third parties. Please retain your own copies for your records.
